SF CAN DO

SF Can DoMany families in poverty are involved in multiple systems of both supportive services (CalWorks, mental health, etc.) and protective services (probation, child welfare removals, etc.). Currently, each of those systems develops a plan for each member of the family, detailing the actions they must take to avoid sanction and/or preserve the benefits provided by that agency. In some cases a single individual may have 2-3 different plans for different issues they face and a single family may have 6-7 plans across all of its members. SF CAN DO (Communities, Agencies, and Neighborhoods Deciding as One) brings together all of those agencies with members of the family to develop a single coordinated case plan that works with and for the family as they individually and collectively strive to achieve their goals. San Francisco is developing this breakthrough model in conjunction with Family Justice, a family based justice program from New York.
